Job Title: Social Worker Transitions Team (Community Learning Disability)
Location: Swansea, Wales
Contract Type: Temporary (initial 3-month assignment)
Working Pattern: Full-time (37 hours per week), Hybrid
Pay Rate: £38.43 per hour

Role Overview:
Sanctuary Personnel is currently recruiting on behalf of Swansea Council for a qualified Social Worker to join the Transitions Team within Adult Services. This post sits within the Community Learning Disability Team and is offered on an initial three-month temporary contract.

The role focuses on supporting young people with learning disabilities as they transition from Children s Services into Adult Social Care a critical period that requires a sensitive, coordinated, and person-centred approach.

Key Responsibilities:

  • Carrying out needs-led assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014
  • Developing person-centred care and support plans in partnership with individuals and their families/carers
  • Liaising with Children s Services to ensure smooth and supported transitions
  • Attending safeguarding and welfare visits as required
  • Maintaining accurate, up-to-date case records on the WCCIS system (experience preferred)

Working Arrangements:

  • Hybrid working model: part-home working, part-office based (Guildhall, Swansea), and face-to-face client visits
  • Flexibility required to travel locally for community visits

Candidate Requirements:

  • Degree in Social Work and registered with Social Care Wales
  • Experience working with adults or young people with learning disabilities, preferably in a transitions setting
  • Knowledge of the legislative framework relevant to adult social care in Wales
  • Familiarity with the WCCIS system is advantageous
  • Enhanced DBS on the Update Service (or willingness to obtain)
