Job Title: Social Worker Transitions Team (Community Learning Disability)
Location: Swansea, Wales
Contract Type: Temporary (initial 3-month assignment)
Working Pattern: Full-time (37 hours per week), Hybrid
Pay Rate: £38.43 per hour
Role Overview:
Sanctuary Personnel is currently recruiting on behalf of Swansea Council for a qualified Social Worker to join the Transitions Team within Adult Services. This post sits within the Community Learning Disability Team and is offered on an initial three-month temporary contract.
The role focuses on supporting young people with learning disabilities as they transition from Children s Services into Adult Social Care a critical period that requires a sensitive, coordinated, and person-centred approach.

Key Responsibilities:
- Carrying out needs-led assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014
- Developing person-centred care and support plans in partnership with individuals and their families/carers
- Liaising with Children s Services to ensure smooth and supported transitions
- Attending safeguarding and welfare visits as required
- Maintaining accurate, up-to-date case records on the WCCIS system (experience preferred)
Working Arrangements:
- Hybrid working model: part-home working, part-office based (Guildhall, Swansea), and face-to-face client visits
- Flexibility required to travel locally for community visits
Candidate Requirements:
- Degree in Social Work and registered with Social Care Wales
- Experience working with adults or young people with learning disabilities, preferably in a transitions setting
- Knowledge of the legislative framework relevant to adult social care in Wales
- Familiarity with the WCCIS system is advantageous
- Enhanced DBS on the Update Service (or willingness to obtain)
