Pay Rate: £26.00 per hour

• Manage a community caseload of adults with diverse needs, including physical disabilities, older age, and complex long-term health conditions.

• Conduct robust, holistic Care Act (2014) assessments and design creative support plans centered around independence.

• Lead on statutory Safeguarding Adults inquiries, ensuring vulnerable citizens are protected while promoting their autonomy.

• Collaborate closely with local health partners, occupational therapists, and housing teams to provide seamless integrated care.

• A qualified, Social Work England-registered Social Worker.

• Proven experience working within a statutory Adult Social Care framework (Community, Integrated, or Locality teams preferred).

• A solid understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols.

• A passionate advocate for independence who can communicate warmly with service users and build fast rapport.
