PRIMARY PURPOSE OF THE JOB:Taking whole case responsibility to provide high quality, outreach and home based Early Help support to children and families using a ?whole family? approach (level 2 and 3) Delivering targeted support to children across age ranges (0 -25 ) and their families in order to prevent the need for statutory services and to ensure families access Universal services Between ages 19-25 relates only to Adults where the SEND Code of Practice is applicableDIRECTLY RESPONSIBLE TO:Early Help Consultant Social WorkerDIRECTLY RESPONSIBLEFOR:THIS POST IS SUBJECT TO A DISCLOSURE AND BARRING SERVICE (DBS) CHECK AT THE APPROPRIATE LEVEL:Enhanced DBSMAIN AREAS OF RESPONSIBILITY:1. Act as case holders for complex cases that step down from children?s social care (level 3/2). This will involve completing early help assessments, arranging Team Around the Family meetings and supporting families to improve outcomes for their children by taking on the role of the Lead Professional and associated tasks.2. Offer support on cases that become more complex and reach level 4. Support is continued on these cases to allow a smooth transition between early help and social care.3. Provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.4. Assess risk and need through early help assessments on cases that step down from Careline. Where appropriate adopt the role of Lead Professional to use common processes for children and families who require more support than universal services including the Early Help Assessment Tool (EHAT), Team Around the Family (TAF) and Lead Professional (LP) role.5. Formulate Team Around the Family plans that reduce risk and need. Take the lead for the coordination of professionals and services to implement the plan and take the lead for reviewing the plan at the required timescales.6. Use de-escalation methods to reduce challenging behaviour from adults and children and young people in families.7. OFSW?s will align their work to the Signs of Safety (SOS) model ? which is underpinned by solution focused methods promoting a whole family approach and use the Family Outcomes Star (or similar) to gauge and demonstrate progress towards positive outcomes.8. Support families/carers in developing practical parenting strategies which enhance positive parent/carer-child relations. This can include facilitated meetings between family members.9. Keep effective records on the electronic systems, which includes the recording of plans, assessments, chronologies and associated general notes.10. Attend regular supervision sessions with the Consultant Social Worker and provide evidence that work is progressing and that plans are improving outcomes for children and young people.11. Visit children, young people and families in their home environment to provide targeted support to improve outcomes.12. Meet the requirements set out in the Early Help Hub Practice Standards.13. Support the delivery of supervised contact to cases that are open to Early Help.14. Develop the City Council?s commitment to equal opportunities and promote anti discriminatory practices in all aspects of work undertaken.15. Promote the safeguarding of children and young people in accordance with statutory guidance and legislation.16. The post holder may also be required to carry out, as necessary, any other duty deemed to be commensurate with the council?s framework for the care and protection of children.17. Make use of Information Technology as required by the City Council.19. Attend staff meetings, reviews, planning meetings and conferences and contribute towards plans and objectives for children and young people.20. Make use of all training and developmental opportunities and demonstrate responsibility towards own professional development.21. Contribute to evaluation and development of services and new ideas by sharing knowledge about theory, skills and practice with other Children?s Services staff, professional groups and interested bodies.22. Establish working relationships with service users and actively engage them through consultation, participation and partnership. Ensure that service users views are always sought and recorded.23. Maintaining an up to date knowledge through training, reading and supervision of the legal framework and responsibilities relevant to the requirements and location of the post.24. Work within a performance management framework to carry out duties to the highest standard of professional practice, consistent with all of the City Council?s policies, code of practices, vision, aims, values and principles.It must be understood that every employee has a responsibility to ensure that their work complies with all statutory requirements with Standing Orders and Financial Regulations of the City Council and to ensure that all work functions are undertaken in accordance with health and safety legislation, codes of practice and the City Council?s safety plan.This job description is not intended to be either prescriptive or exhaustive.
